Also known as breast implants or augmentation mammoplasty can change the size and shape of your breasts. Some women may desire fuller breasts, while others may seek to reverse the effects that pregnancy, breast feeding or massive weight loss can have on their breasts. Breast augmentation is usually performed by inserting a saline or silicone gel breast implant behind breast tissue or behind the breast muscle.

 

Two Types of Implants      
There are two types of implants — silicone breast implants and saline (salt water) breast implants.
Silicone gel implants are smoother and softer than saline implants. Many women prefer the softer; more natural-feel of silicone implants especially those with little breast tissue. Silicone-gel implants are also less likely to ripple than saline breast implants.

Surgical procedure      
Breast Implant Incisions
The Incision location is based on individual anatomy.
- Fold incision / inframammary Incision
  is placed near the crease where the breast and chest come together.
- Nipple incision / periareolar Incision
  is placed along the outline of the areola, or the brown or pink pigmented region
  surrounding the nipple.
- Armpit incision / transaxillary Incision
  is placed in the natural folds of the skin of the armpit.

 


Best Position for Breast Implant Placement

Subglandular implant placement places the implant under the breast tissue, but above the muscles in the chest.

 


Complete submuscular implant placement places the breast implant under/behind the pectoral and chest muscles.

 

 

Pre Operative Assessment for Size and Placement of the Implant      
The goal is to augment your breasts to a size in better proportion to your physique and maintain a natural look.
If you have only a small amount of breast tissue and want to enhance the tissue much as possible, it is recommended that an implant is placed below the muscle in order to prevent the feeling or appearance of the implant. There is also less chance of capsular contracture.
If you have large chest muscles as a result of exercising, the implant should be placed above the muscles in order to allow for comfortable movement and a natural look.
For patients with mild to moderate breast sagging, larger implants placed over the muscle is recommended.          


Results      
Breast augmentation increases breast size and may also balance your body proportions. Clothes may fit better, and you may also experience an increase in confidence and self-esteem
            
Duration of Operation:

After Care      
All dressings and bandages are removed on the third day following the operation.
The patient is requested to gently move the implants upward, downward, and inward in order to decrease the chances of capsule contracture.
Only gentle massaging is recommended. Overly aggressive manipulation of the implant or strong squeezing can possibly cause an inflammation, thus producing an adverse effect on the capsules.
During the first month following surgery, you should exercise your implants three to four times per for fifteen to twenty minutes. After that, if breast contour improve satisfactorily, you will need to exercise only once or twice daily.


Recovery Time:      
Patient need to stay in Bangkok 7 more days after surgery for post-op cares.

Post Operative Care      
For at least three to five days upon surgery, try to keep arm extension to a minimum. After removing the gauze dressing, you may be required to wear a support bra to help your breasts form and maintain a pleasing and balanced shape. You will probably be able to resume exercise and normal physical activities within a month or two when soreness has subsided.
You are able to fly back home after seven days.

Surgical procedure

There are many types of techniques but the most common procedure involves an anchor-shaped incision by following the natural contour of the breast.

The incision outlines the area from which breast skin is removed and defines the new location for the nipple. When the excess skin has been removed, the nipple and areola are moved to the higher position.

 

Stitches are usually located around the areola in a vertical line extending downwards from the nipple area, and along the lower crease of the breast. Some patients, especially those, who have small breasts and minimal sagging, may be candidates for modified procedures requiring less extensive incisions. For example, one such procedure is the "doughnut (or concentric) mastopexy," in which a circular incision is made around the areola, and a doughnut-shaped area of skin is removed. If patients having an implant inserted along with their breast lift. The surgeon will make a pocket directly under the breast tissue or deeper under the muscle of the chest wall.

 

Results

Your surgeon will make every effort to make your scars as inconspicuous as possible. Still, it's important to remember that mastopexy scars are extensive and permanent. They often remain lumpy and red for months, then gradually become less obvious, sometimes eventually fading to thin white lines. Fortunately, the scars can usually be placed so that you can wear even low-cut tops.

After Care

After surgery, patient will be wrapped in elastic bandage or a gauze dressings or surgical bra. A small tube may be placed in each breast to drain blood and fluids for the first day or two. The extent of the post-operative swelling and bruising is depending on whether you tend to bruise or swell easily. The amount patient can expect vary for each individual but past surgeries or injuries should be a good indication. Applying cold compresses or ice packs will reduce swelling and relieve discomfort. Many patients use a watertight plastic sandwich bag filled with frozen berries or peas. Regular icing is the key to relieving swelling. The bandages will be removed a day or two after surgery, though patient will continue wearing the surgical bra around the clock for several weeks, until the swelling and bruising subsides. In most cases, stitches will be removed in one to three weeks. Sometimes the surgeon will use only dissolvable sutures. If breast skin is very dry following surgery, moisturizer can be applied several times a day, but the suture area must be kept dry at all times.

 

Recovery Time

The recovery takes one to two weeks.

The best candidates for a breast reduction are women who are physically and emotionally healthy.Results are usually achieved in-patients with small sagging breasts. Breasts of any size can be lifted but heavy breasts may not last long. If the patient is planning to have more children, it may be a good idea to postpone your breast lift but there are no special risks that affect future pregnancies. For example, mastopexy usually doesn't interfere with breast-feeding.

Usually breast reduction is performed for physical relief rather than simply cosmetic improvement. Very large breasts that restrict their activities and cause them physical discomfort trouble most patients who have the surgery.

 

Breast reduction surgery is sought by many patients to relieve the physical and painful strain on neck, shoulder and back caused by excessively heavy breasts. A strain that can be so severe that it leads to chronic headaches, Back aches and deformities, shoulder indentations breathing problems and a variety of other medical problems. Breast reduction (reduction Gynecomastia) removes fat, glandular tissue, and excessive skin, resulting in smaller, lighter and firmer breasts that are better shaped in proportion to the rest of the body.

 

Male Breast Reduction (Gynecomastia)

Although rarely discussed, enlarged male breasts are a common condition. About 40 to 60 percent of men are affected with enlarged breasts.  There is no known cause for gynecomastia this, although there has been some correlation with various drugs or medical conditions.

Men who feel self-conscious about their appearance are helped with breast-reduction surgery. The procedure removes fat and/or glandular tissue from the breasts, and in extreme cases it removes excess skin, resulting in a contoured chest that is flatter and firmer.

 

Surgical procedure

Reduction Gynecomastia typically is performed to reduce disproportionately large, sagging breasts. It also can reduce the size of the areola (the dark pink skin around the nipples )

The most commonly technique, called brassiere pattern skin reduction, involves both horizontal and vertical incisions that follow the contour of the breast. The vertical incision creates a key hole-shaped pattern above the areola.

 

Working through the incisions, the surgeon excises excess tissue, fat and skin on the sides of the breast. Skin is removed from the area within the vertical incision and around the areola. The nipple, areola and underlying tissue are moved through this space to a new higher location. Another technique involves completely detaching the nipple from the breast before relocating it. Since a transplanted nipple loses all sensitivity, this approach is performed only when deemed necessary for patients with extremely large breasts.

 

After the nipples are repositioned, skin on both sides of the breast is moved down and around the areola and then brought together to re-contour the breast.

Sutures close the wounds under the breast and around the nipple area. Following surgery, a gauze dressing may be applied to the breasts or the patient may be placed in a surgical brassiere.

Depending on the extent of the surgery, the procedure usually lasts two hours or longer.

The patient must be willing to accept the change from large uncomfortable breasts without scars to small comfortable breasts with scars. Scars remain highly visible for a year following surgery then fades to some degree.

Since incisions were made around and below the nipples, scars should not be noticeable even in low-cut-clothing. To permit proper healing, you should avoid excessive exercise and refrain from overhead lifting for three to four weeks. The decision on when to return to work and normal activities depends on how fast you heal and how you feel.

 

It is important to follow a careful plan on return to strenuous activity, in order to avoid tension on the wound (which could lead to worse scarring).

You are able to fly back home after seven days.

Keep your bandage clean and dry. Sponge bathe only. You will remain in bandages for about one week. Light activities can be resumed within a few days, although your incisions might swell. Routine physical activity and exercising (specially lifting, pulling) should be avoided for at least 6 weeks.

NO SMOKING for 2 weeks before and 4 weeks after surgery.
